When can I apply–is there a certain time of year when I can enroll?
You can enroll any time of the year. Coverage becomes effective the first of the month after you enroll. |

When does the health insurance renew?
All of the plans renew every January 1st no matter when you sign up. |

How long am I required to be a member?
You must make an initial commitment of three years.. |

What happens if I want to terminate my insurance coverage before my three-year commitment ends?
If you terminate early, you will lose the capitalization fee you paid when you joined the cooperative.

Why do I have to sign up for three years?
The three-year commitment is required by state law, and allows the cooperative to negotiate a more favorable arrangement with the insurer that includes caps on premium increases, bringing you more stable rates. It also protects the cooperative from the adverse selection that has occurred in other purchasing pools where enrollees could enroll and disenroll at any time.

What forms are required for me to enroll?
You must complete an application, health questionnaire, co-op membership form and USDA form. These will all be provided to you and are available on this website at the bottom of each page.

What options do I have for paying my premiums?
If you are a farmer, we can bill you monthly or quarterly.You may also choose to pay monthly premiums through direct withdrawal from your checking or savings account, or out of your milk check. If you are an agribusiness, you will be billed monthly.

Can I be rejected for coverage if I have a current medical condition?
No, you cannot be rejected for coverage. However, your final premium rates are based on your and/or your family’s health status.

Do you cover pre-existing conditions?
Pre-existing conditions are covered if you have had one year or more of continuous health insurance coverage, without a lapse of more than 63 days from the expiration date of your previous coverage and the effective date of your FHCW coverage.
